Evolv3 Consulting LLC Coaching Services Disclosures Agreement
This document outlines key aspects of the coaching relationship between you and Evolv3 Consulting LLC, ensuring clarity and mutual understanding as we embark on this journey together.
1. Nature of Coaching:
Coaching is a partnership focused on developing personal and professional goals and strategies. It's a thought-provoking process designed to maximize your potential.
2. Limitation of Services:
Coaching does not include therapy, counseling, or consulting and is not a substitute for professional mental health care or medical treatment.
3. Confidentiality:
Confidentiality is fundamental to our relationship. Evolv3 Consulting LLC adheres to the ICF Code of Ethics regarding confidentiality. Please note that coaching is not legally recognized as a confidential relationship like medical or legal professions.
4. Client Responsibilities:
You are responsible for the decisions and actions you take during coaching. You are free to accept or reject suggestions from Evolv3 Consulting LLC and you bear full responsibility for your well-being.
5. Cancellation and Rescheduling Policy:
A fee of $100 will be assessed for cancellations, no-shows, or rescheduling made within 24 hours of an appointment.
6. Payment Terms:
Payments must be made in accordance with the terms specified on Evolv3 Consulting LLC’s website https://learning.evolv3consulting.com or by any written agreement between you and Evolv3 Consulting LLC. All payments are subject to the terms of our no-refund policy as set out in the Terms & Conditions at https://learning.evolv3consulting.com/terms-and-conditions.
7. Coach's Commitments:
Evolv3 Consulting LLC is committed to your success, maintaining professionalism, respect, and support throughout all sessions. Evolv3 Consulting LLC will prepare for each meeting to ensure your time is well-spent.
8. Release of Information for ICF Credentials:
Your information may be used for credentialing verification with ICF, including names, contact information, and coaching start/end dates. No personal notes will be shared.
9. Recording of Sessions:
For group coaching and courses, all sessions will be recorded. For individual coaching sessions, you have the option to request a recording.
10. Data Protection and Privacy:
Evolv3 Consulting LLC is committed to protecting your personal information. Please refer to Evolv3 Consulting LLC ‘s Privacy Policy for details on how Evolv3 Consulting LLC handle your data.
11. Agreement to Terms:
